Who is entitled to a refund for the purchase of a wheelchair?

Any person insured or reported for insurance as a family member.

Where can you get such an application?

Applications from the National Health Fund are written by a specialist doctor: orthopedist, surgeon, neurologist,  family doctor, specialist in the field of physiotherapy.

Where to go for a stroller?

You can pick up the stroller at a medical store or pharmacy. 

​

What refund amounts?

Refund amounts are unchanged and  are a supplement to the purchase value of the stroller. The difference between the price of the cart and the value of the surcharge (600)  must be covered by the buyer . Example: The regular price in the store  for a cart is PLN 1,000 per cart. - National Health Fund refund PLN 600 ---- Customer surcharge PLN 400.

P - 116  - 500 pln (unfortunately, the Ministry of Health has planned a 30% patient payment)

( ) - if the pillow costs PLN 500, you have to pay 30% of PLN 500, i.e. PLN 150.
